Nashville-based Main Street Health on Sunday announced it has raised $315 million in new capital as it seeks to expand into 26 states.

The company, founded in 2021, is a value-based health care company primarily focused on rural areas. Main Street places health navigators in rural primary care clinics with the goal of improving care coordination and communicating with patients. According to a release, early results from Main Street health navigators include a 23 percent reduction in hospitalizations and a 9 percent reduction in medical costs.
